Diabetes is a serious threat to human health, long-term poor blood sugar control can lead to blindness, kidney failure, cardiovascular accidents, amputation and other serious complications, and even ketoacidosis or unconscious hypoglycemia.China is the No. 1 diabetes country with 140 million patients, about 40 million of whom rely on insulin injections for life.Severe patients whose blood sugar is difficult to control can only be effectively treated by transplantation therapy that extracts islet tissue from the donated pancreas and injects it minimally, thus avoiding the progression of complications.However, due to the severe shortage of donors and the complexity of islet separation technology, the current clinical needs are difficult to meet.Therefore, how to regenerate human islet tissue in vitro on a large scale has become a worldwide academic problem and has received widespread attention.
Professor Yin Hao, director of Shanghai Long March Hospital Organ Transplantation Center, said that after more than 10 years of research, the research team reprogrammed patients' blood PBMC into autologous iPSC cells.The technology has matured and is a major breakthrough in regenerative medicine for diabetes.
The first beneficiary of the technology was a 59-year-old male with a 25-year history of type 2 diabetes who developed terminal diabetic nephropathy (uremia), which appeared in June 2017 and received a kidney transplant.However, due to the near-exhaustion of its islet function, insulin injections are required several times a day, and there is a great risk of serious complications of diabetes in the future
On July 19, 2021, due to concerns about hypoglycemia and the adverse effects of poor blood sugar control on the long-term survival of the donor kidney, the patient underwent autologous regenerative islet transplantation at Shanghai Long March Hospital.
Yin Hao said that since then, the patient's fasting and C peptide after meals have been significantly improved compared with before surgery, confirming that the islet function has been restored effectively.Overall, this is the first international human tissue replacement therapy using autologous islet to treat T2D patients with impaired islet function.
Long March Hospital revealed that in addition to the first case of severe type 2 diabetes, the above-mentioned joint team has also successfully carried out and cured multiple cases of brittle type 1 diabetes.Yin Hao said that the future team will focus on stem cell-derived regenerative islet tissue research, further developing "universal" regenerative islet tissue without immunosuppression, so as to provide a new cure for diabetes patients who rely on insulin injections.
