A small clinical trial has found that a technique known as deep brain electrical stimulation (DBS) improves cognitive performance in patients with traumatic brain injury. Data from the trial, published Dec. 4 in Nature-Medicine, showed that five participants completed cognitive tests 15 to 52 percent faster after three months than before receiving DBS. For some participants, the improvement was transformative, even years after the injury.One of the authors, neurosurgeon Jaimie Henderson of Stanford University, said. Moderately severe traumatic brain injury (msTBI) is usually caused by head trauma, which can lead to neuronal death and disconnection of brain circuits, leading to long-term cognitive difficulties.People with such injuries are often unable to restore their pre-injury lives and jobs, and more than 5 million people in the United States have MSTBI. MsTBI is thought to affect neural circuits in the thalamus, which is near the center of the brain and is associated with attention, decision-making and working memory.Henderson and colleagues hope to apply electricity to key parts of the thalamus — a technique that has been shown to activate and restore connections between damaged neurons — to improve cognitive function in patients with MSTBI. To precisely target neurons in the thalamus, the team used individual brain imaging data and specialized anatomical maps to design personalized treatments for four men and one woman who participated in the study. All participants experienced msTBI for at least two years, and they implanted electrodes near the outer side of the thalamus in both cerebral hemispheres.After the surgery, the researchers spent 14 days fine-tuning the stimulation parameters of each participant.They then used the implants to apply current at 150-185 Hz for 12 hours a day for a total of three months. To test participants' cognitive function, the researchers used a test to evaluate task transformation, attention, and working memory, in which participants had to connect consecutive numbers or letters arranged in specific geometric patterns. After three months, the results showed that participants performed 30.7 percent better on average on attentional tasks than before surgery and 32 percent faster at completing cognitive tests. Nicholas Schiff, a neuroscientist at Cornell University and one of the authors of the paper, said the team hopes to continue the work by conducting larger trials and developing a reliable protocol to train other centers to provide treatment
