About 15% of colorectal cancer patients are malmatched repair defects, which are characterized by DNA malmatched repair defects.Because of their high immunogenicity, these patients respond well to immunotherapy. At present, immunotherapy represented by PD-1 monoclonal antibody has become the standard treatment for patients with colorectal cancer.
The original purpose of the new adjuvant therapy is to improve the survival of cancer patients as an adjunct to surgical treatment.However, with the development of immunotherapy, it may be used as a radical treatment for patients with colorectal cancer with early mismatch repair.The study shows that colorectal cancer patients who achieved complete tumor remission after new adjuvant immunotherapy and received observation and waiting strategies did not recur or metastasis.
"While new adjuvant immunotherapy shows a high rate of complete remission for malmatched colorectal cancer, there is still a lack of biomarkers for immunotherapy responses to identify which patients in malmatched colorectal cancer can achieve complete remission."Deng Yanhong introduced that clinically this part of the patient to achieve drug cure, exemption from surgery and organ reservation, need to be identified before treatment.
In response, the team compared the tumor-immunity characteristics of patients who received 6 cycles of NAI and incomplete remission to identify biomarkers of NADT.
Based on samples from PICC patients, the team used NGS sequencing techniques to analyze genomic and transcriptome characteristics of patients with complete and incomplete remission.In addition, the team integrated single-cell RNA sequencing data and multiple immunofluorescence data to explore potential mechanisms for complete mitigation of response-related immune cells.The results showed that the difference in tumor immune characteristics existed before treatment in patients with complete remission and incomplete remission, including T cell receptor, antigen presentation and so on.In addition, patients with complete remission had higher CD8+ T cell density.
This study reveals that immunodiagnosis in colorectal carcinoma and molecular markers of complete remission induced by immunotherapy may help to reduce the cost of immunotherapy, reduce immune-related adverse reactions.
