Swiss scientists have improved walking and balance in a person with severe motor impairment due to Parkinson's disease by using nerve implants for targeted epidural stimulation.These studies highlight the potential for using neural prostheses to treat motor disorders in Parkinson's patients.The results were recently published in Nature-Medicine.
About 90% of patients with advanced Parkinson's disease develop motor disorders, including gait disorders, balance problems, and frozen gait attacks.These impairments reduce the quality of life, increase the severity of related diseases, and existing treatments are not very effective.Lumbosacral spinal cord-specific epidural electrical stimulation (EES) modulates neural activity that controls movement, and recent studies have shown that it can restore standing and walking function in people paralyzed by spinal cord injury.
Gregoire Courtine of the Swiss Federal Institute of Technology and Jocelyne Bloch of the University of Lausanne Hospital have collaborated to develop an EES-based nerve prosthesis to restore natural activation of disturbed leg nerves when walking in Parkinson's patients.After extensive testing of the neural prosthesis in nonhuman primates, the researchers began their first human trial of the method, a 62-year-old male with Parkinson's disease for 30 years, with medication and deep brain stimulation.
The researchers first developed a customized anatomy of the spinal cord region targeted by the EES to guide precise surgical implantation of nerve prostheses.The patient's wireless sensor was then used to detect movement intentions, triggering EES to activate the leg nerves to produce natural gait.The results showed that the nerve prosthesis improved the subjects' walking and balance disorders, and he further reported significant improvements in quality of life (the patient has now been using the prosthesis for about eight hours a day for two years).
These preliminary results suggest that targeted EES may be a promising treatment option for common mobility disorders in Parkinson's patients.But this is a proof-of-concept study in which only one participant is involved and further research is needed in larger clinical trials to verify the effectiveness of this approach.
