A special set of immune cells in the intestines may play a key role in controlling the progression of Crohn's disease.
Crohn's disease is one of two major forms of inflammatory bowel disease (IBD), which affects about 1 per cent of adults in the UK and the US altogether.However, little is known about the cause of the disease, says Adrian Hayday of the Francis Crick Institute in London.
Immune cells in the gut are thought to play a role, Hayday said, especially in a group of cells called δTT cells.
He and colleagues wanted to better understand what these cells looked like in IBD patients compared to healthy bowel patients.To do so, they took wall samples from 150 people undergoing colonoscopy, some of whom had IBD.

They found that the number of V-gamma-4 (Vg4) cells, a particular immune cell subgroup in IBD patients, was usually lower than in healthy people in the gut.Hayday says the cells are mainly present in the intestinal lining.
But this is not the case. People with fewer Vg4 cells are more likely to develop IBD.On the contrary, especially for Crohn's patients, small studies have found that people with fewer immune cells in their intestines may develop more serious diseases.
In people with reduced Crohn's disease, Vg4 cells are less likely to recur within the next five years, similar to those with healthy intestines.
"These cells won't stop you from getting the disease, but they'll give you a better response."It's kind of like a vacuum cleaner: if you have a good vacuum cleaner, you can control everything," Hayday says.”
It's unclear why these cells appear to be depleted in patients with IBD, but they may serve as biomarkers to help doctors more specifically diagnose what type of IBD a person has, he said.
"When patients come to me at the clinic, there is no clear biomarker to tell us which drug is best for them.Robin Dart of King's College London, a member of the team, said the cells could help doctors determine if a patient has IBD that is likely to recur.
